In this installment of medical billing of claims through electronic means using NSF 3.01 specifications, we're going to continue our review of the EA1 record, picking up with field number 14.EA1 field 14, positions 177 - 184, is the admission date 2 field. This date needs to be filled in if the patient was readmitted to the hospital or facility for any reason at all relating to the condition being billed even if it is just as a follow up.EA1 field 15, positions 185 - 192, is the discharge date 2 field. This date need to be filled in if the patient was readmitted and then discharged for any reason at all relating to the condition being billed even if it is just as a follow up. If the patient was readmitted but not discharged yet, then this field is left blank.EA1 field 16, positions 193 - 207, is the supervising provider ID. This is the ID number of the supervising provider if there was one overseeing the actual physician performing the services. This is usually the administrator of the hospital or facility but can be any supervisor.EA1 field 17, positions 208 - 222, is reserved for future use. The length of this field indicates that it will most likely be used for an ID number of some kind.EA1 fields 18 - 20, positions 223 - 255, are the supervising provider name fields for first, last and middle initial. This must be the legal name as registered with the facility.EA1 field 21, positions 256 - 257, is the supervising provider code. This is a two character code that indicates the type of supervising provider that it is. Consult your manual for a list of valid codes.EA1 fields 22 - 24, positions 258 - 290, are the EMT or paramedics last, first and middle initial. This needs to be filled in if the patient was taken to the facility in an emergency and was treated by an EMT or paramedic on the way there.EA1 field 25, positions 291 - 298, is the date care assumed field. This is the date that permanent care of the patient began, if this is the case. This is usually the date then a patient is admitted into a care facility such as a nursing home.EA1 fields 26 - 29, positions 299 - 318, are additional diagnosis codes fields. Normally, there are only three or four diagnosis codes needed for sending a claim to a payer. But with the increased number of diseases and conditions and contributing factors to patient problems, it has become necessary to provide more fields for transmitting additional codes. These four fields however, are seldom used.EA1 field 30, positions 319 - 320, is filler national and must be left blank or the claim will be denied.This concludes our review of the additional claim data, related mostly to facility information.
