Medical Billing - GA0 Record Fields 1 Through 12

In our last installment on medical billing, we presented an overview of G records, which are certificates of necessity that need to be transmitted to the payer for certain types of services to prove that the need for the patient was there. In this installment we will begin detail discussions of each CMN, beginning with the GA0 record, or ambulance certification.

The GA0 record needs to be sent any time a patient is taken to a hospital or facility by ambulance. Most CMNs are sent with information that is either Y or N, or yes or no. We'll begin our coverage of the fields for the GA0 ambulance certification.

GA0 field 1, positions 1 - 3, is the record type and needs to be filled in with GA0.

GA0 field 2, positions 4 - 5, is the sequence number in the claim. Because each claim can have more than one CMN, the sequence number 01, 02, etc., must be transmitted.

GA0 field 3, positions 6 - 22, is the patient ID. This is the same ID that is transmitted in the CA0 record and all subsequent records to identify the patient.

GA0 field 4, positions 23 - 39, is the line item control number. This is the number assigned to that particular line item in the claim.

GA0 field 5, positions 40 - 42, is the patient's weight in pounds. This field must be filled in if the weight is known by either the patient or a relative. Rarely is the weight taken on the spot when a person is being transported by ambulance.

GA0 field 6, position 43, is the hospital admission indicator. This is the first of many Y or N questions and is filled in with a Y if the patient was admitted to the hospital after the ambulance ride.

GA0 field 7, position 44, is the transportation type code. This is a special code that designates just how the patient was transported.

GA0 field 8, position 45, is the bed confined before transport indicator. This is filled in with a Y if the patient was bed ridden before the ambulance ride.

GA0 field 9, position 46, is the bed confined after transport indicator. This is filled in with a Y if the patient was bed ridden after being brought to the hospital or back home.

GA0 field 10, position 47, is the moved by stretcher indicator. This is filled in with a Y if the patient had to be taken to the ambulance on a stretcher.

GA0 field 11, position 48, is the unconscious or in shock indicator. If the patient was either in shock or unconscious when brought to the ambulance, this is filled with a Y.

GA0 field 12, position 49, is the emergency transport indicator. This is filled in with a Y if the patient was brought in via a 911 call to the police.
